尿のNa排泄亢進と細胞外液量減少 (volume depletion) を伴う低Na血症を認める疾患群として、 CSWS、 RSWS、 MRHEが挙げられる。 これらの疾患は尿Na>30mEq Lとなるので、 SIADHと酷似するが、 治療方法が異なるためSIADHとの鑑別が大切になる。 細胞外液量減少がある場合や、 SIADHの治療をしても反応がない場合にこれら3疾患を鑑別に考える。 CSWSでは、 脳損傷・くも膜下出血などの中枢神経疾患によって、 尿からNa排泄が亢進し、 細胞外液量減少をきたす病態である。 罹患後2-10日以内で発症し、 3-4週間で自然に軽快する。 SIADH よりは頻度が低いと言われている。
